A "PET heart test" is a cardiac PET-CT scan. Unlike a routine cardiac workup with ECG or stress echo, cardiac PET addresses specific questions about inflammation, viability after heart attack, or perfusion under stress. This light guide walks through when and how cardiac PET is used.
Three Distinct Cardiac PET Applications
Cardiac PET serves three different clinical questions:
- Active inflammation: FDG PET for cardiac sarcoidosis or myocarditis
- Myocardial viability: FDG PET to identify recoverable heart muscle after MI
- Myocardial perfusion: Rb-82 or N-13 ammonia for stress/rest perfusion
Each uses different tracers, different prep, and answers different questions.
When Cardiac PET Is Ordered
Indications include:
- Unexplained heart block in patients under 60
- Suspected cardiac sarcoidosis
- Heart failure of unknown cause
- Pre-revascularization viability assessment after MI
- Suspected myocarditis when MRI alone insufficient
- Recurrent ventricular arrhythmia without clear cause
- Sudden cardiac arrest survivors with negative initial workup
These are specialized indications. Routine cardiac assessment uses ECG, echo, stress testing — not PET.
The Sarcoidosis Prep Challenge
For cardiac sarcoid evaluation, the prep is elaborate:
- 24 hours of high-fat, low-carbohydrate diet
- 12 hours NPO before scan
- 3 hours before scan: fat load (50 mL heavy cream or 2 tbsp butter)
- Some centers add IV heparin to release fatty acids
The goal: suppress normal myocardial glucose uptake so granulomas stand out. Failed prep = uninterpretable scan.
Patient Experience
Day of scan:
- Arrival, blood sugar check, vital signs
- IV insertion
- Tracer injection (FDG, ~10 mCi for adult)
- 60-minute uptake phase (quiet rest, warm room)
- Scan (15-30 minutes)
- IV removed, hydration, normal activity resumes
Total clinic time: 90-120 minutes. Radiation dose: 4-8 mSv plus CT portion (3-8 mSv).

Comparison with Cardiac MRI

Frequently Asked Questions
Why do I need 24 hours of high-fat diet?
To suppress normal myocardial glucose uptake. Without this, the heart's normal background uptake masks any abnormal inflammation.
Can I have a regular meal the morning of the scan?
No. NPO except water in the 12 hours before. The fat load (3 hours before) is the only intake.
Will the scan show coronary anatomy?
PET doesn't show coronary anatomy directly. The CT portion of PET-CT can show calcified plaque. For full coronary anatomy, CCTA is the test.
Is the radiation safe?
4-8 mSv from PET + 3-8 mSv from CT = 7-16 mSv total. Comparable to a CT abdomen or multi-phase CT. Acceptable for the diagnostic value when indicated.
