Wat screening op vettige lever inhoudt

Leverecho
Abdominale echo is het eerste-lijnsbeeldvormingsinstrument voor het opsporen van hepatische steatose (vetophooping in levercellen). FibroScan (Transiente elastografie met CAP)
FibroScan is een gespecialiseerd echo-gebaseerd apparaat dat twee dingen tegelijk meet:

Leverstijfheid (LSM — Liver Stiffness Measurement): Een trilling wordt in de lever gestuurd, en de snelheid waarmee de golf door het weefsel reist, wordt gemeten. Stijver weefsel = meer fibrose. Dit geeft een schatting van het fibrosestadium (F0–F4 op de METAVIR-schaal), van geen fibrose (F0) tot cirrose (F4) — zonder naaldbiopie.

CAP (Controlled Attenuation Parameter): Kwantificeert gelijktijdig de mate van steatose (vetinhoud) in decibel per meter. CAP gradueert steatose van S0 (geen significante steatose) tot S3 (ernstige steatose), wat een preciezer beeld geeft dan alleen echo.

Bij NAFLD:

FIB-4-score en andere niet-invasieve fibrose-indices

Wie moet gescreend worden op vettige lever

- Leeftijd over 45 met metabolische risicofactoren en geen recente leverevaluatie
- Bekende vettige lever op eerdere afbeelding zonder vervolgbeoordeling van fibrosestadium

What is the difference between NAFLD and NASH?
NAFLD (non-alcoholic fatty liver disease) is the umbrella term for liver fat accumulation not caused by significant alcohol use. Can fatty liver be reversed?
