NHS Targeted Lung Health Check Explained for Patients

The NHS Targeted Lung Health Check (TLHC) program is one of the largest national lung cancer screening initiatives in the world. Eligible patients in participating areas of England are invited to a risk assessment, and high-risk individuals are offered a low-dose CT scan. The program started in 2019 and is steadily expanding. This guide explains who qualifies, what the appointment involves, and the alternatives for those who don't qualify or live outside participating areas.

What the NHS TLHC Program Is

The TLHC is a national programme delivering targeted lung cancer screening to high-risk individuals. Key features:

  • Invitations are sent to people registered with a GP in participating areas
  • Initial risk assessment by trained nurse or healthcare professional (in person or by phone)
  • Eligible individuals offered a low-dose CT scan
  • Findings communicated to patient and GP

By 2026, TLHC has expanded to about 50% of England's population, with continued rollout planned.

Eligibility: Age and Smoking History

To qualify for the initial TLHC invitation:

  • Age 55–74 (in most regions; some areas slightly different)
  • Registered with a participating GP
  • Has ever smoked (currently or in the past)

After initial assessment, eligibility for the actual CT scan depends on the PLCOm2012 risk model, which integrates:

  • Age
  • Smoking duration and intensity
  • Time since quitting
  • BMI
  • Education level
  • Family history of lung cancer
  • COPD diagnosis
  • Ethnicity

A score above a defined threshold (typically 1.5% 6-year cancer probability) leads to an LDCT.

Booking and What to Expect

The patient pathway:

  1. Invitation letter or phone call from your GP or TLHC team
  2. Initial risk assessment (telephone or in-person, ~30 minutes)
  3. If eligible: LDCT scan booking at a participating site
  4. LDCT scan (~15 minutes, no contrast needed)
  5. Results within 2 weeks; further imaging if positive
  6. Follow-up scans if Lung-RADS 3 or 4

The risk assessment includes questions about smoking history, family history, and current symptoms. No need to fast or change medications.

Risk Assessment Phase

The PLCOm2012 risk model is used. Sample calculations:

  • 65-year-old male, 1 pack/day for 30 years, no family history, no COPD: typically eligible
  • 60-year-old female, ½ pack/day for 20 years, family history of lung cancer in mother: typically eligible
  • 56-year-old male, 1 pack/day for 10 years (started age 46), quit 4 years ago: borderline, depends on other factors
  • 70-year-old female who quit 25 years ago after 1 pack/day for 5 years: likely not eligible (low pack-years, long quit)

The model is more sensitive to combinations of risk factors than simple pack-year thresholds.

LDCT for High-Risk Patients

If the risk assessment shows you're high-risk:

  • LDCT is scheduled within 2–4 weeks
  • Modern 1.5T or higher LDCT scanner; typical effective dose 1.0–1.5 mSv
  • Lung-RADS category assigned to any findings
  • Annual repeat LDCT if Lung-RADS 1 or 2; 6-month follow-up if 3; further workup if 4

Private Alternatives When Outside NHS

For people who don't qualify for TLHC or live in non-participating areas:

  • Private LDCT in UK: £200–400 cash at private radiology centers (Bupa, Spire, Nuffield)
  • International self-pay: mainland China ¥1,200–2,500 ($170–360); India INR 5,000–10,000 ($60–120); Thailand THB 3,500–7,000 ($100–200)
  • DIY combined screening: book LDCT plus other indicated screening on a 3–5 day medical trip

For UK-resident smokers who didn't qualify for TLHC (e.g., too young, light pack-year history with risk factors), self-pay private screening is increasingly common.

Patient Pathway for Expats and Visitors

For expats and international visitors who want lung screening matching NHS-style eligibility:

  • Eligible smokers in their home country: typically self-pay in mainland China or Asia
  • Family history without typical eligibility: self-pay screening common
  • Recent return from heavy smoking who didn't formally qualify: self-pay

The Chinese self-pay option offers the same LDCT scanner type at 1/5 to 1/10 the US private cost, with English-language reports and convenient booking.
