Choline PET-CT was the imaging workhorse for prostate cancer recurrence for about a decade — from the early 2000s until PSMA PET emerged in the late 2010s. Choline retains a role in select scenarios, particularly when PSMA is unavailable or for certain salvage radiation planning. Choline Biology in Prostate Cells
Choline is a building block for cell membrane phospholipids. Cancer cells with high membrane synthesis (rapid proliferation) take up choline more rapidly than normal cells. In prostate cancer:
- Choline kinase is upregulated
- Choline uptake correlates with tumor cell density
- Recurrent cancer cells continue this pattern even at small tumor volumes
Choline PET uses either C-11 choline (20-minute half-life, very short) or F-18 fluorocholine (110-minute half-life, easier to ship). Both detect the same biology.
Indication: Biochemical Recurrence
The main indication for choline PET (historically):
- Rising PSA after radical prostatectomy or radiation therapy
- Failure of conventional CT and bone scan to localize disease
- Localization for salvage radiation planning
PSMA PET (where available) is now the preferred test for this indication. Choline is used when:
- PSMA is unavailable locally
